For Certified Documents:

Signature and Initials:

  • Must be signed by the Authorized Officer.
  • On documents with more than one page, each page must be initialed by the Authorized Officer, and all pages must be numbered (e.g., page 2 of 5).

Annotation on First Page:

  • Annotate the first page with the statement: “I have sighted the original document and certify this to be a true copy of the original.”

Details to Include:

  • List the following details:

    • Occupation or profession (including relevant identification number)

Stamp or Seal:

TRANSLATING DOCUMENTS

The translator must provide a signed statement affirming the accuracy of the translation and indicating that they belong to an approved organization in the country where the translation is done.

This statement should include:
